Request for Provider

Providers are encouraged to log in to Care Choice to search requests for providers.

Provider Certification

The Ohio Department of Developmental Disabilities (DODD) is waiving the application fee for all initial applications for independent providers.

Settings Review

If you are opening a new Adult Day or Vocational Program or have moved the location of your program, a site visit is required prior to being added to the provider list. To schedule this, contact Denise DeVault, Director of Planning and Development at ddevault@blanchardvalley.org or 419-422-6387.

MUI/UI

If a person with a disability, whether child or adult, is abused, neglected, or exploited, please report this information as soon as possible to the Hancock County Board of Developmental Disabilities.  If you have an unusual incident that you believe meets the level of MUI, you should submit it to uir@blanchardvalley.org.  During evenings, weekends, or holidays, call our emergency number at 419-722-9262.

All providers who are contracted, certified or licensed to support persons with developmental disabilities are required to report certain MUIs to the county board within four hours of the incident. These include:

  • Accidental or suspicious death

  • Exploitation

  • Misappropriation

  • Neglect

  • Peer to peer act

  • Physical abuse

  • Sexual abuse

  • Verbal abuse

  • Media inquiries

The most important thing you can do is make sure the person is safe.  Only by working together as a community can we help ensure the health and safety of people with disabilities.

Waivers

Waivers are one way to pay for services that support individuals with developmental disabilities living on their own, with family, with a roommate, or with a provider through Ohio Shared Living.  The department administers three different waivers: the Individual Options Waiver, the Level One Waiver, and Self-Empowered Life Funding Waiver.

Background Checks

Blanchard Valley Center offers background checks free of charge for all providers in Hancock County.
